Position Overview
We are seeking a caring and skilled Licensed Practical Nurse to provide quality nursing care to clients in their homes. As an LPN with Scensob Home Care, you will work under the supervision of a Registered Nurse while delivering hands-on care that improves the health and well-being of our clients.
Key Responsibilities
Direct Patient Care:

Key Responsibilities
Clinical Practice:
- Conduct thorough initial evaluations and ongoing assessments of patients’ physical status
- Develop individualized, goal-oriented treatment plans based on evaluation findings
- Implement therapeutic interventions including therapeutic exercise, manual therapy, gait training, balance training, and functional mobility training
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Recommend and train patients in the use of adaptive equipment and assistive devices
- Modify treatment plans based on patient progress and response to therapy
- Provide wound care education and positioning recommendations as appropriate
Documentation & Compliance:
- Complete comprehensive documentation in accordance with Medicare guidelines and company standards
